A Report on the Multi-Axis Trainer (M.A.T.) Incident
Introduction
This official report on the Multi-Axis Trainer (M.A.T.) Engineering Assessment aims to provide a detailed account of the incident involving a young girl and to present preventive measures based on thorough investigation and evaluation.
Description of the Incident
According to Mr. Scotty Wong's engineering assessment, the Multi-Axis Trainer experienced motor failure on March 26, 2020, at approximately 9:45 a.m. The failure was attributed to water droplets entering the trainer due to small cracks in the roof above. These cracks, caused by falling branches during a storm, allowed water to seep through, compromising the machine’s functionality. CCTV footage captured the entire incident, providing clear evidence of the events as they unfolded.
As a result, an 11-year-old girl, Miss Rey Liu, became trapped upside down in the trainer for around 30 seconds. The experience caused her significant distress, and she complained of a severe headache due to her inverted posture. At 10:15 a.m., she was taken to Dr. Xixi Yang for a medical evaluation. As a safety measure, Dr. Yang contacted her parents, and the child was transported to Waglan Memorial Hospital for further examination. Fortunately, no health issues were identified after thorough observation.
Future Recommendations
Although the girl did not sustain any serious injuries, several precautionary measures have been proposed to prevent similar incidents in the future. Mr. Scotty Wong has recommended sealing the micro cracks in the roof with silicon sealant. In addition, Miss Ochoa has advised trimming the branches of the trees located above the M.A.T. building to prevent further damage. These recommendations have been approved by the Management Committee and will be implemented promptly in coordination with the relevant parties.
